Actually the output stages of power amps are often bootstrapped, with the
resistor (4k in the example) split into two resistors, and a largish
capacitor connected from the split point to the output. Every time the
output goes to one extreme, the capacitor pushes the bootstrap point to go
above or below the supply voltage thus maintaining the current thru the
