Depends on what performance you want; an efficient design is hard once the
voltage gets high enough.
But, yes, it does..
The mechanical construction takes priority over almost any other parameter -
f.ex. the core size will be determined by the isolation distance, not power,
and the core shape is driven by the former available...
A couple of kV @ 100 W is easy enough. Above 5 kV is trouble and tens of kV
takes real thought.
What you really have to watch is the inter-layer capacitance in the HV
winding and the switching losses i.e. capacitance must be minimised. One way
is to wind each layer of windings in the same direction and connect them in
series with spacers between, there are coil formers for this under "Gas
Ignition transformers f.ex.". CCFT transformers is another place to look for

avoid the huge losses caused by ringing of the secondary when driven by a

Like any standard transformer - Stuff like the resonant frequency etc. I
